This form is for members of College Park Church Castleton who are planning on resigning or transferring their current membership. We care deeply about each of our members and want to help you unite to another gospel preaching church as soon as possible. Please have every individual member who is transitioning out of College Park Church complete the form below.

Please note: This form does not automatically remove you from membership. Our elders and covenant members will be involved in finalizing the process and you will be notified when your removal is finalized.
